[Tier 2] How to follow up on Deactivation Requests

  1. Ensure that Care appropriately verified the member's identity
  2. Open the member's account in the Care App
  3. Remove payment, uncheck both SMS boxes, place all plans on-demand, and confirm there are no side-effect messages from the RN in Care Center.
  4. In the member's Personal Info tab, click Deactivate Member AccountScreen_Shot_2021-05-13_at_4.06.07_PM.png
  5. Select the appropriate reason for deactivating the member's account. Add the ZD Ticket link as a comment in the deactivation request

  6. Send the deactivation macro to the member via Email
